Medication Information & Required Forms for the 2026–2027 School Year

As we prepare for the upcoming school year, please review the following important information if your child will require medications at school.

Required Forms
Please complete all required health and medication forms over the summer prior to the 2026–2027 school year. These forms must be updated each year to ensure we can safely support your child’s health needs at school.

• Parent Request for Administration of Medication by School Personnel (PDF)

  • For daily, PRN (as-needed), or asthma medications.

• LISD Allergy Action Plan for Elementary Students (PDF)

  • For students with life-threatening allergies.

• TEA Seizure Action Plan (PDF)

  • For students with a seizure diagnosis.

Medication Reminders

• All medications must be in the original, properly labeled container.

• Please check expiration dates- medications must be valid through the school year.

• Inhalers, EpiPens, and emergency medications should be clearly labeled and in working order.
